Post-Acute Liaison- Senior Living - Now Hiring

Salary range:
$70, 928 to $104,000/annualExempt position with bonus structure Preferred Schedule:
Sunday through Thursday Under general supervision, the Post-Acute Liaison is responsible for generating business from referrals sources to meet the community's revenue and census goals.
The majority of this position is spent in outside sales; developing relationships with community hospitals, physician offices, community health clinics and other community outreach programs.
Essential Functions:
Engage in occupancy growth, partnership development and integrated health system strategy work.
Maintain a log of daily external strategy work; report out on external activities at strategy meetings.
Collaborate with ED, NHA, DON, Medical Director, DOR and other key team members to maximize competencies in short-stay rehab program with ongoing development of niche and specialty programs to align with strategic physician and health system partnerships.
Provide leadership team with insights and observations from external strategy work to help the team understand the trends and changes occurring in the acute and post-acute healthcare environment.
Meet with patients and family at hospital to educate on short-stay program and benefits while building trust in a seamless transition to post-acute.
Foster strategic relationships and alignment with assisted living, home health and other community based service providers.
Utilize data analytics and outcome performance measures to demonstrate competencies and value in accountable partnerships.
Interpret financial reports and support desired payer mix budget goals.
Develops and fosters partner relationships with hospitals and key physician groups.
Understands and supports accountable care partnerships.
Interacts with others by developing and maintaining rapport, demonstrating interpersonal sensitivity toward others to establish a relationship and continue the admission process.
Makes daily hospital rounds, keeping in contact with case managers, discharge planners and Hospitalists to maintain relationship and referrals.
Responds to all callers and visitors inquiring about admissions in timely manner as established by the Health Services Administrator.
Pre-assesses potential patients so that clinical efforts expended are in proportion to their urgency and appropriateness for the community.
Works with the health care team to identify resident care needs in order to assure that adequate resources and services are provided to residents, meeting regularly with health care team to assure highest practicable care will be delivered.
Prepares marketing reports regarding referral statistics.
Ensure high level of integrity of referral data within Company computer system.
Contributes to planning, research and writing of the community's occupancy development plan for strategy and tactics.
Maintains working knowledge of, and assist in, accessing community resources.
Responds to requests for information and brochures in a timely manner and according to performance standards outlined by Health Services Administrator.
Receives inquiries by telephone or fax from case managers, discharge planners, or patient/family.
Requests copies of information from hospitals, SNF or other health care facilities of prospective resident.
Supports admissions internally every other weekend and for vacation coverage as well as increased referral times.
Knowledge:
Current understanding of Medicare and Managed Care environment with alternative payment systems, value based healthcare, risk bearing payment systems and narrowing post-acute networks.
Policies and procedures for procuring assistance for residents; Federal and State statues to ensure compliance with regulations; Knowledge of Title XXII, OBRA Knowledge of Medicare A & B, Managed Care Insurance, co-insurance, Medicaid and ACO partnerships.
Marketing Theory & Application.
Contracts for continuing care (HC, AL, RL).
Skills:
Strong communication, organization, negotiation and public relation skills Effective problem solving and decision making skills Effective verbal and listening Proficient with the PC including the MS suite of products Time management skills Proficiency to communicate complex information in way that can be understood by varying degrees of health literacy.
Can relate equally well when holding a conversation with a family member or consultant surgeon.
Minimum Requirements:
Education - Bachelor's Degree preferred, Therapist (PT, OP, or SP), LCSW, Nurse, MPH, or Marketing Specialist.
Experience/Training - Two or more years in a related field that demonstrates the ability to work in a long term care setting, preferably with geriatric residents, admissions and a marketing background Medical or business office education or training desired, Clinical license such as RN, LPN, RT, IT or SLP preferred.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
